Multidisciplinary care can prevent heart failure in people at risk, prolong survival, reduce hospital admissions and improve quality of life. This module explores the effective use of multidisciplinary care for heart failure management, including the preparation of short and long-term management plans and referral to other health professionals.
By completing this learning activity, participants will be able to:
- Understand the components of multi-disciplinary care for HF patients.
- Coordinate care with other health professionals and provide appropriate patient information.
- Generate short and long term care plans for HF patients involving the multi-disciplinary care team.
- Understand the patient-centric approach to care planning.
